There are a number of services that are not covered by the Medical Services Plan, but may be covered by your Extended Medical. Payment for uninsured services is due upon receipt of service. We accept the following forms of payment: Cheque, Debit, Mastercard, Visa.

  • School/Work time off, Government UIC form, Parking form, Handidart Application

  • Employer's Work Capability form

  • Nursing Home Registration / Administration

  • Reports / Letters

  • Handwritten note / letter

  • Medical Advice by letter

  • Driver's License Exam

  • Participatory Fitness for Sports Team / Camp

  • Industrial First Aid - limited exam

  • CPP / MOT / Insurance Co.

  • Excision Mole (cosmetic)

  • Transfer of Records

  • Physician Review of Records – for transfer of Medical/Legal (per 15 min)

  • Transfer of Records - photocopying - per page (first 10 / subsequent pages)

  • Missed Appointment Charge

Nurse Services

We have a team of nurses that assist the doctors. The nurses are available Monday to Friday, 8am to 4:30pm.

  • Suture or staple removal

  • Lifestyle and Chronic Disease Prevention Education

  • Immunizations

  • Injections

  • Ear Syringing

  • Holter Monitor

  • Blood pressure monitoring

  • Liquid Nitrogen treatment

  • Same day or Urgent appointments for: cough, sore throat, bladder infection, ear infection, and other minor ailments
